Steps to Recreate:
Navigate to Corporate → Templates → Add Trigger Template
Enter a value into the Name field.
Select any of the three available Type options.
Observe that the Name field is cleared immediately after selection.
Expected Result:
The Name field should retain its value regardless of the Type selection order.
Actual Result:
The Name field is cleared when a Type is selected, regardless of which option is chosen.
Additional Notes:
Issue consistently occurs for all three Type options.
Suggestion: Reorder the form fields so that Type appears before Name, prompting users to select the Type first. This would prevent the confusion of a cleared Name field after the type entry.
